Asthma Action Plan
An Asthma Action Plan, such as the one provided by Northern Medical Centre aids the patient in developing an in-depth understanding of the symptoms of asthma, and provides instructions for a response plan.
As stated by the National Asthma Organisation, the “Use of a written asthma action plan:
- Reduces absences from work or school
- Reduces hospital admissions
- Reduces emergency visits to general practice
- Reduces reliever medication use
- Improves lung function.”
Our asthma plans are uniquely tailored to each patient and allow personalised health care. These written plans provide information on recognising exacerbations (flare-ups), and provide steps on responding to the situations.
A typical plan may include basic doctor details, patient’s name and doctor’s contact information. They can also include patient carer information, danger signs for patient symptoms, treating exacerbations, medical advice and a response plan for urgent situations.
Peak expiratory flow (PEF) information can also be recorded on the action plan, to benefit those with more severe symptoms.
